Learning Journey Report

This report documents a year-long, community-driven collaboration in which more than 550 San Franciscans partnered with researchers to identify and address barriers to advance care planning (ACP). Through community-led workgroups and pilot events, participants developed culturally resonant initiatives that increased engagement and laid the foundation for sustained community leadership in ACP.

"We've Got to Bring Information to Where People Are Comfortable" - Journal of the American Geriatrics Society

This study concludes that community-based ACP events designed and led by and for the Black community were highly acceptable and effective. Findings highlight the importance of financial planning as part of ACP and the role of Black-owned businesses as trusted spaces for these conversations.

"Advocating for What We Need" A CBPR Approach to Advance Care Planning in the Latinx Older Adult Community - Journal of the American Geriatrics Society

This study describes a feasible, effective, and culturally tailored ACP intervention. Co-developed community events offered a promising approach to reducing disparities in ACP among Latinx older adults.

"At the End I Have a Say": Engaging the Chinese Community in Advance Care Planning - Journal of Pain and Symptom Management

This study concludes that community-developed, intergenerational events that emphasized the value of ACP and addressed cultural barriers proved acceptable and effective in increasing ACP engagement within the Chinese community.
