Learning Journeys

In 2020, the Palliative Care Work Group (PCWG) embarked on a community-led research process to assess the landscape of Advance Care Planning (ACP) and Palliative Care, and to understand the reasons for health inequities in access among Black, Chinese and Latin people in San Francisco. This research journey was designed, led, and implemented by community leaders, and engaged hundreds of community members. Read about our journey and what we learned here. Community Grantmaking

The San Francisco Palliative Care Workgroup and the Stupski Foundation are sponsoring a Community Grantmaking Initiative. Following on the Learning Journeys work, the Stupski Foundation is providing funding and support to Black, Chinese, and Latin Community Grantmaking Committees.

Made up of community members, the Committees will collectively determine the process for selecting grantees, select the grantees, and decide on funding amounts for each grantee. The grantmaking will focus on community-based education, awareness, and acceptance of serious illness care services (palliative care, advance care planning, caregiver support, and social services) for people living in San Francisco within each of the three communities. We anticipate that this will be a 3- to 6-month project for each community.

社區設立撥款
